Elation Electronic Health Record (EHR)

The Student Health Clinic's medical practice has transitioned to an Electronic Health Record (EHR). All medical patients seen at the Student Health Clinic will need to create an Elation Electronic Health Record account.

Once an appointment has been scheduled, you will receive an email to your Hawkeye email address with an account entry link to "Elation EHR".

Once an account has been created in Elation Electronic Health Record (EHR), students can:

  • Complete demographics, medical history intake questions, health screening questionnaires, and consents.
  • Access their ElationPassport and view their medical records including their office visits, medications, treatment plans, and educational handouts.

All Health Sciences program students and RedTail Athletes will have an Electronic Health Record.

If campus is closed, the Student Health Clinic is also closed. This includes weather-related closings. See the Academic Calendar for scheduled college closings.

Appointments are required. Please call or email to schedule an appointment.

The Nurse Practitioner and School Counselor are also available via Tele Health.
