Students, visitors, and/or staff who are injured while on campus should contact Public Safety at 319-296-4234 no matter how minor the injury may be.

If it is an emergency, contact 911 first and then Public Safety.

A Student/Visitor Report of Injury form [PDF] needs to be completed by the injured person as they may be covered under workers compensation insurance.

  • Students working on-campus in a practical training or clinical rotation as required by their program of study who are injured (e.g. laceration by equipment) should file a student report of injury.

  • Students working off-campus in a practical training or clinical rotation as required by their program of study who are injured (e.g. a needle stick) should immediately notify the off-site clinic or business which they are working for. The student also needs to contact their program coordinator immediately and follow the student report of injury instructions.

The Student/Visitor Report of Injury form must be sent within 24 hours of the injury to Barb Farrell at barbara.farrell@hawkeyecollege.edu.

Transportation To and From the Medical Treatment Facility

All transportation to and from the necessary medical treatment facility is the responsibility of the student/visitor and will not be covered or provided by Hawkeye or its employees.

In the event that an injury, including minor, or illness occurs as a result of performing normal job responsibilities, please follow these instructions.

If the injury is serious and the employee needs to be transported to the hospital

  1. Call 911 for an ambulance.

  2. Call 319-296-4234 for Public Safety.

  3. Call Barb Farrell at 319-296-4409 to notify her of the injury.

  4. Within 24 hours of the injury, the injured employee or their supervisor must complete the Employee Report of Injury form [PDF], and submit it to Barb Farrell, Operations office, to comply with the mandatory reporting requirements of the State of Iowa.

  5. Occupational Health should be notified that the claim is for a work injury and will be paid by Hawkeye Community College, not the staff member's health insurance carrier.

If the employee needs medical attention, but the injury is not serious enough to go to the hospital

  1. The injured person must see a doctor at Unity Point/Allen Occupational Health Services, the designated provider for Hawkeye Community College.

  2. Call Barb Farrell at 319-296-4409 to authorize the appointment at Unity Point/Allen Occupational Health Services.

  3. Within 24 hours of the injury, the injured employee or their supervisor must complete the Employee Report of Injury form [PDF], and submit it to Barb Farrell, Operations office, to comply with the mandatory reporting requirements of the State of Iowa.

  4. Occupational Health should be notified that the claim is for a work injury and will be paid by Hawkeye Community College, not the staff member's health insurance carrier.
